The first step in our consultation process is to submit the form below or call our office directly. * Note – it is very helpful for you to submit a series of photos with your consultation request form (see below).

Once we have received your consultation inquiry, our office will reach out to your directly and help you schedule your consultation. Consultations are performed either in-person, or virtually via our HIPAA-Secure video conferencing app.

The next step in our consultation process is a detailed initial phone consultation with our Team of Patient Care Coordinators. During this meeting, you and your personal Coordinator will:

• Discuss your goals for surgery
• Analyze and review your photos
• Review your medical/surgical history
• Go into detail regarding Dr.Locketz’s unique techniques and experience
• Review downtime and recovery
• Begin to look at potential dates for surgery

This initial phone consultation will also:

• Help you develop a comprehensive surgical plan to accomplish your goals
• Educate you regarding expectations for surgery and recovery
• Provide you the most accurate estimates on surgical pricing

The next step in our consultation process is a private, in-person or virtual consultation with Dr. Locketz. During this consultation, Dr. Locketz will perform a detailed examination of your facial anatomy and dive deep into your aesthetic goals. Together, you and Dr. Locketz will optimize your surgical plan and begin to prepare for the next phase in your aesthetic journey.
